Silent Systems, Loud Burden Healthcare systems around the world continue to face rising pressure from chronic diseases, infectious disease threats, injuries and growing healthcare expenditure. Yet despite broad recognition of the value of prevention, most health systems remain focused on treating illness rather than preventing it.
Developed by FTI Consulting in collaboration with the Future Investment Initiative (“FII”) Institute, Silent Systems, Loud Burden explores a critical challenge facing healthcare leaders globally: while we can measure health outcomes, there is currently no comprehensive way to assess whether countries and health systems are truly prepared to prevent future health burdens before they emerge.
The report introduces a conceptual framework for measuring Preventive Health Readiness and lays the foundation for a forthcoming Preventive Health Readiness Index, which will be launched at the Future Investment Initiative in Riyadh in October 2026. The Index aims to provide policymakers, investors, healthcare leaders and international institutions with a practical tool to assess preventive health capabilities, identify system gaps and support more informed decision-making.
Key Insights From Silent Systems, Loud Burdens
Prevention remains under-prioritised Health systems continue to spend significantly more on treating illness than preventing it, despite strong evidence that preventive interventions improve outcomes and reduce long-term costs.
There is a global measurement gap While countries track health outcomes and individual prevention initiatives, there is currently no comprehensive framework for assessing whether health systems are equipped to prevent future health burdens.
Readiness can be measured The report introduces a structured framework spanning policy and governance, funding and incentives, data and digital capabilities, access and capacity, and health outcomes.
The Index will turn insight into action The forthcoming Preventive Health Readiness Index is designed to help policymakers, investors and health system leaders identify strengths, vulnerabilities and opportunities for targeted investment and reform.
This initiative reflects the collaboration between FTI Consulting and the Future Investment Initiative Institute, combining FTI Consulting’s healthcare strategy expertise with FII Institute’s commitment to advancing solutions that create positive impact for humanity.
Together, the organisations are working to develop a practical, evidence-based approach to measuring preventive health readiness and supporting more resilient, sustainable and future-ready health systems. The Preventive Health Readiness Index will be unveiled at the Future Investment Initiative in Riyadh in October 2026, providing a new lens through which policymakers, investors and healthcare leaders can assess preparedness, benchmark progress and identify opportunities to strengthen prevention systems around the world.
